Description (Service) Support, information and education for people with Alzheimer's disease or related dementia and their caregivers, families and general public * home or office visits, referrals and ongoing follow up * monthly support group meetings are held in Oshawa, Whitby, Ajax, Pickering, Port Perry, Uxbridge, Bowmanville and Beaverton for family caregivers * early stage dementia support programs for persons living with dementia * dementia education and brain health presentations and workshops * training provided for residential facilities, community agencies, students and other community organizations * MedicAlert Safely Home program is available * resource centre with books, videos and information circulars* First Link program connects clients and families to support & services by accepting self-referrals and referrals from community partners, family physicians and nurse practitioners * Primary Care Collaborative Memory Services * social and recreational programs including choir, arts/drama, Minds in Motion, Walking Buddies and Brain Wave Caf├ęs * Blue Umbrella program provides dementia customer service training for local businesses and others
