Description & Services
Description (Service) HOME SUPPORT PROGRAM 
Services that support adults and their caregivers with needs related to aging, physical and/or mental health * assists clients to remain in their own homes and communities, and to function as independently as possible  
 
Home Support Services  
 
Transportation  
Drives for clients primarily to medical appointments, and when time allows to shopping and errands * for Clarington clients, a specially designed wheelchair accessible van is available 
 
Home Help 
Light housekeeping assistance  
 
Home Maintenance 
Occasional minor repairs, yard work, grass cutting, snow shovelling or one-time clean-up  
 
Meals on Wheels™  
Nutritionally balanced meals to people who are unable to prepare meals or easily shop for groceries * hot or frozen meals are delivered by volunteers * special diets are available  
 
Luncheon Out  
With a focus on helping seniors stay connected, provides a nutritious meal and entertainment in a social setting 
 
Friendly Visiting  
Volunteer visits on a regular basis with lonely or isolated clients * activities may include playing cards, taking walks, doing crafts or just enjoying pleasant conversation  
 
Telephone Reassurance  
Daily Monday to Friday telephone calls to clients for a friendly check-in and brief chat * checks on their well-being and reassures them that help is available if and when needed  
 
Foot Care  
Community Clinics provide foot care by registered practical nurses * regular appointments are booked for clients at each site
